Life Infusion Medication Therapy Management Program

The Life Infusions Medication Therapy Management (MTM) program is a comprehensive review of a patient's drug regimen. Developed with input from Life Infusion staff pharmacists and practicing physicians, the Life Infusion's MTM program is designed to identify, resolve, and prevent medication related problems to achieve optimal outcomes of drug therapy.

Program Features:

Life Infusion pharmacists deliver educational services for the chronic diseases with specific focus on oncology, osteoarthritis, solid organ transplant, multiple sclerosis, crohn's disease, hepatitis - C, rheumatoid arthritis, psoriasis, hemophilia, pain, transplant, and infectious disease. 

Our clinical pharmacy MTM services include:

1. Review the patient's medication profile for:

  • Therapeutic duplication
  • Drug-drug interactions
  • Drug-disease interactions
  • Contraindications
  • Adverse effects
  • Sub-therapeutic response
  • Overutilization or Underutilization
  • Potential fraud and abuse (e.g., repeated requests for lost medication)
  • Multiple prescribers
  • Other parameters as determined by the pharmacist on a case-by-case basis

2. Provide one-to-one education to the patient regarding appropriate medication use


3. Utilize interventions to improve adherence to prescribed medications regimens such as directed counseling and/or special packaging and/or refill reminders


4. Contact the patient's prescriber as necessary to recommend changes to the medication regimen


5. Document all interventions on the Life Infusion's MTM Intervention Form and submit all necessary information to the patients plan sponsor for integration with the patient's care team.
